Using the Connect N’ Care app, your patients can report their symptoms through our built-in evidence-based guidelines, and have a HIPAA compliant video visit or phone call with the provider. Your patients can utilize the built-in personal health record capabilities to manage their medications, be reminded to take medications and be alerted to adverse drug interactions.
The physician can easily diagnose, treat and prescribe from the convenience of their mobile device or PC remotely. Upon receiving notification of a new patient case, the assigned clinician can log on using the Connect N’ Care app or web-based portal and review the SOAP notes, then proceed to do a quick video consult so they can “lay eyes” on the patient. At the end of the consult, the telehealth encounter can then be documented in the EMR using API’s, HL7 or FHIR protocols.
